Uninsured Medical Services

Some physician services provided by our office are not covered by OHIP (Ontario Health Insurance Plan) and do require payment by our patients, out of pocket, for rendering of the service. Below is a list of common services not covered by OHIP and their cost. Our office accepts cash, cheque and e-transfer for payment. The email for e-transfer for payment is: payfredmed@gmail.com.

Driver’s Medical Examination and Form $150
Disability Tax Credit Form $85
Note for Massage/Orthotics/Physiotherapy etc $30
All Other Forms / Attending Physician Statement Minimum $40

Treatments / Injections:
Ear Syringing* (When Not OHIP Covered) $30
TB Skin Test (1 Step) $40
TB Skin Test (2 Step) $80
Wart Cryotherapy* (When Not OHIP Covered) $20
Skin Tag removal Minimum $50

Missed Appointments:
Missed Routine Appointment $40
Missed Mental Health Appointment $80

Miscellaneous:
Fax prescription refill (without visit) $25
